NOTE: There are two locations/cities for these meetings. JEITA is hosting the workshop in Kamakura. SEMICON Japan and JEITA are co-hosting the public conference in Makuhari, Chiba. JEITA will provide transportation from Kamakura to Makuhari hotels on Tuesday evening.

Meeting Registration Form and Instructions

NOTE: Enter your information on the registration form using one of these options:

(1) DOWNLOAD /EMAIL -- Download the form as a pdf file to your computer using the "SAVE AS" command [usually a mouse right-click]. Enter your information electronically, check to make sure your information is correctly presented on your form, and email as an file attachment to sarah.mangum@sematech.org.

(2) PRINT AND FAX/ MAIL -- Print the form and enter your information. Fax to the ITRS attention at 512.356.7118. This is a shared fax so be sure and use a cover sheet to identify as "ITRS."

HOTEL INFORMATION for KAMAKURA AND MAKUHARI
**ATTENTION PLEASE READ**

During SEMICON Japan, most hotel rooms in the Makuhari area are booked by travel agencies. It is very difficult to get a room due to these room shortages this season. JEITA and SEMI Japan have made special arrangements to secure rooms in both areas:

JTB (JTB Global Marketing & Travel Inc.) is the contact for hotel reservations for BOTH Kamakura and Makuhari visits. To take advantage of the special handling by JTB and to secure a room at the Makuhari with these arrangements, you MUST have a room at the Kamakura Prince Hotel [see exceptions below].
